Arizona Cardinals legend Roy Green is probably best known for his speed that got him the 4.2 time in the 40 yard sprint. In fact, his out of this world speed and world class skill landed him the nickname “Jetstream” and earned him the respect of the greatest players across the league. Many consider him to be the greatest Cardinal of all time. Unfortunately, Roy encountered serious health issues since hanging up the cleats for good. “I had some health issues for years including three heart attacks and two strokes. I definitely wasn’t sleeping — two hours max in the evening and I’d wake up and be up for another couple hours just back and forth,” Green said during an interview. “When you don’t sleep well, you’re just miserable the next day.” It wasn’t since teaming up with David Gergen, President of Pro Player Health Alliance and Executive Director of the American Sleep and Breathing Academy (ASBA), that Roy Green learned the health complications he had were most likely caused by his sleep apnea. Roy mentioned he finally got tested, got the oral appliance and likes to joke by saying he now has more energy than he had when he was playing. The results of receiving treatment for his sleep apnea were shocking to Roy who says, “I haven’t had any serious health issues since getting treated five years ago. Now Dr. Mark Castle, who recently achieved Diplomate Status in the ASBA, manages my treatment.” The overwhelming positive effects of the oral appliance caused Roy Green to shift his focus to helping improve the health of current and former professional athletes through promoting sleep apnea awareness across the country. After joining the cause and using his extensive amount of connections to players, he has helped get over 1400 former players tested and/or treated for sleep apnea. Now Mr. Roy Green dedicates his time and efforts to the American Sleep and Breathing Academy and Pro Player Health Alliance. Making visits to members’ practices and attending all ASBA functions have become a part of his routine, which is why the ASBA is proud to offer congratulations to Roy Green after it was recently announced that he will be entering the Arizona Cardinals Ring of Honor.
ASBA members, you need to be heard! This is a great opportunity to weigh in and make a difference. This is an email I received from Congressman Marty Russo. If you want Oral Appliance Therapy to have its place with Planes, Trains, and Automobiles (what a funny movie that was) we need to get on this. On a serious note, this is a great opportunity to weigh in and share your expertise with influential policy makers. Federal Motor Carrier Safety Administration: We saw 16 really solid comments which are clearly from ASBA folks (including several of you). This is 6% of the total of 274 comments. Many of the other comments are not from experts so it’s more impactful than the small percentage suggests. But more comments will clearly strengthen your case tremendously. Federal Railway Administration: There are only 8 comments from ASBA folks – a subset of the same comments mentioned above. This means 8 of the FMCSA comments were not repeated on the Federal Railway Administration regulation. You may want to remind people to make sure they comment on both sites – and if they left out FRA, go back in and make the comment. On the FRA, there are only 26 total comments so far. That means ASBA folks represent 31% of total comments!! There is real opportunity here to totally dominate the comment field if more comments are made. The purpose of this bulletin is to remind healthcare professionals on FMCSA’s National Registry of Certified Medical Examiners (the National Registry) of the current physical qualifications standard and advisory criteria concerning the respiratory system, specifically how the requirements apply to drivers that may have obstructive sleep apnea (OSA). As an update, the DOT has announced three “public listening sessions” as shown below. My strong recommendation is that you try to organize a presentation by someone at each of these- find several strong presenters who are willing to attend any of these sessions in DC, Chicago and LA. Not the same person – good if there is as many as will help. I’ve attached the federal register announcement with more details. Summary FMCSA and FRA announce three public listening sessions on May 12, 17, and 25, 2016, to solicit information on the prevalence of moderate-to-severe obstructive sleep apnea (OSA) among individuals occupying safety sensitive positions in highway and rail transportation, and of its potential consequences for the safety of rail and highway transportation. FMCSA and FRA (collectively “the Agencies”) also request information on potential costs and benefits from possible regulatory actions that address the safety risks associated with motor carrier and rail transportation workers in safety sensitive positions who have OSA. The listening sessions will provide interested parties an opportunity to share their views and any data or analysis on this topic with representatives of both Agencies. The Agencies will transcribe all comments and place the transcripts in the dockets referenced above for the Agencies’ consideration. The Agencies will webcast the entire proceedings of all three meetings. Dates The listening sessions will be held on: Thursday, May 12, 2016, in Washington, DC; Tuesday, May 17, in, Chicago, IL; and Wednesday, May 25, in Los Angeles, CA. All sessions will run from 10 a.m. to noon and 1:30 p.m. to 3:30 p.m., local time. If all interested parties have the opportunity to comment, the sessions may conclude early. As a result these superstar athletes then become more than just patients, but every day people who publicly support and spread the awareness of OSA at events across the nation. It is expected that this event will draw over 5,000 thousand attendees. The American Sleep and Breathing Academy’s annual meeting in Scottsdale, which took place April 15th and 16th, is the only meeting for dental and medical professionals alike where NFL legends, congressmen and some of the most successful sleep experts in the country come to share their experiences and knowledge with members who attend. This unique combination offers an outlook on all aspects of sleep medicine from the patient’s point of view to working with local government to promote awareness and recognition of sleep disordered breathing in the local community. The meeting started off with a returning key note speaker, former Congressman, Marty Russo. Using his background in his excellent lobbying expertise, Russo spelled out what dentists can do to be successful in influencing the outline of the proposed rulemaking and how oral appliance therapy can be used for the Federal Motor Carrier Safety Administration and the Federal Railway Administration. By addressing the questions covering areas of most interest to federal policy makers that closely align with the doctor’s expertise, dentists can impact guidelines that will be used to regulate treatment protocols relating to individuals with OSA occupying safety sensitive positions in highway and rail transportation. Dr. Rod Willey, the founder and CEO of the Koala Centers for Sleep Disorders, gave his most heartfelt speech to date. Willey is among the most productive sleep apnea dentists in America and it is clear that his driving force, behind all of this success, is helping to save as many people as possible. This was evident as his passion spewed into the audience, giving attendees tips for building a successful practice while also helping their community. Dr. Jay Wright, orthodontist, had his public speaking debut at the ASBA annual conference. Dr. Wright had no idea but Dr. Paul Serrano, considered by many to be the finest orthodontist in the country, was in attendance. Dr. Serrano said that Dr. Wright was spot on with his lecture. He gave perhaps the finest pediatric sleep algorithm to date. Dr. Kent Smith gave a great presentation on the three things he wishes he knew before treating sleep apnea. Following his lecture, he received a standing ovation and later in the evening, Dr. Smith was informed that he won the ASBA presidency for 2017-2018. David Gergen and NFL greats presented Dr. Elliot Alpher and Dr. Harry Sugg with initiation into the American Sleep and Breathing Academy’s Hall of Fame for their vast achievements in dental sleep medicine. Dr. Sugg, not knowing the honor was coming, broke into tears as Eric Dickerson expressed his gratitude towards him. Dr. Kevin Mueller did a masterful job presenting how to reconstruct a person’s occlusion using crowns, showing that, with proper restorative dentistry and a qualified lab, an oral appliance might not be necessary. Dr. Mueller uses Hershberger Dental Lab in Phoenix, Arizona exclusively. Following Dr. Richard Bonato’s presentation there was a line of people eager to speak with him regarding the information in his speech. Dr. Bonato is heavily involved in the medical devices industry and gave an informative talk on micro recorders and how they can measure patient treatment compliance. Mack Newton, inspirational speaker, several time Tae Kwan Do world champion, spoke about discipline, formulating a good strategy and plan and when to start. Newton said that the “when” is the most important part and now is as good a time as any. Newton also said that if something is possible then it is inevitable and that is how he has always lived his life. Following his presentation, Mack was greeted with a standing ovation. Dr. Robert Iversen, is the program director of Arrowhead Health Centers which offers patients a unique and easy way at obtaining care. Arrowhead Health is a one stop shop for patients who can receive care from multiple specialists under one roof; a convenience not usually available to patients at a reasonable price. Dr. Iversen presented that the doctors in his facility are working with ASBA trained dentists across the community to get oral appliances prescribed to their patients. He also touched on the fact that, skilled dentists, working within the medical model, have a standardized fee but those dentists that do not work within the medical model have a significantly higher fee and have hurt the industry overall. For example, companies like Sleep Services and Mediscore are hurting the industry when they bill over $9000 for an oral appliance and $7000 for an AM Deprogrammer (splint). In addition, these dentists are not properly trained in dental sleep medicine and it is quite evident that patient outcome is not their primary concern. For these reasons, and more, Dr. Iversen asserted that the ASBA is extremely important because it teaches dentists to work within the medical model and to steer clear of fraudulent business people. Dr. Patricio Reyes, board certified and world renowned neurologist for the VA, asserted that the CPAP machine is widely over prescribed. Dr. Reyes touched on the treatment delays within the VA and how he hopes that oral appliance therapy will play a bigger role with the vets and enable care to be more quickly available. Current ASBA President, Dr. Erin Elliott, gave the sleep medicine newbies an incredible introduction to sleep. Attendees were fortunate enough to get this insight from a skilled and professionally trained dental sleep specialist. The second breakout session was presented by Angela Prayne from the Koala Center for Sleep Disorders. She gave insight on front and back office winning strategies for a successful sleep practice. Dr. Gerry Weisfogel, who is the nation’s first cardiologist to be boarded and certified in sleep, talked about chronic care management and how to implement programs with networks of care providers. This breakout session was attended by many and Gerry gave the doctors tips for chronic care with Medicare patients in order to generate more oral appliances.

Executive Directors: Mr. David Gergen CEO, David Gergen, CDT and President of Pro Player Health Alliance, has been a nationally respected dental lab technician for over 25 years. He received the award for “The Finest Orthodontic Technician in the Country” given by Columbus Dental in 1986. He also has been appointed Executive Director of the American Sleep and Breathing Academy Dental Division, a national interdisciplinary academy dedicated to sleep training and education with over 60,000 members. David rolled out of bed on December 4, 1982 and had his career “ah ha” moment. He knew he was going to be an orthodontic technician and he knew he was going to help people all over the country to help treat their sleep disorders in partnership with their dentists. He has worked for some of the pioneers in the orthodontic and sleep dentistry fields. He was the personal technician for the likes of Dr. Robert Ricketts, Dr. Ronald Roth, Dr. A. Paul Serrano, Dr. Clark Jones, Dr. Harold Gelb, Dr. Joseph R. Cohen, Dr. Rodney Willey, Dr. Allan Bernstein, and Dr. Thien Pham. One of his proudest achievements is receiving The National Leadership award for Arizona Small Businessman of the Year in 2015 Mr. Alan Hickey CFO Alan Hickey was appointed Executive Director of the American Sleep and Breathing Academy “ASBA” in 2015. His mission is to integrate dentistry and medicine in the specialty of sleep medicine disorders. The ASBA develops education and training for sleep healthcare practitioners. Along with the clinical journal ‘Sleep Diagnosis & Therapy’ the ASBA publishes a consumer-orientated magazine titled ‘Sleep & Wellness’ which is distributed to over 127k dental and medical practices. The ASBA also hosts an annual convention in April every year for healthcare specialists. President: Dr. Kent Smith Dr. Kent Smith is a Diplomate of the American Board of Dental Sleep Medicine and is on the Advisory Committee of the Australasian Academy of Dental Sleep Medicine. In 2007, he was asked to start a sleep curriculum at the Las Vegas Institute of Advanced Clinical Studies. In 2011, he started the SKISH Initiative (“School Kids in Safe Hands”), to keep bus drivers and students safe on the roads. In 2012, he was the first in Texas to be awarded a Dental Sleep Medicine Facility Accreditation and is the creator of the “Best Sleep Hygiene” app, which is a free download. That same year, he was asked to host a radio show on sleep disorders, and has been the host of “The Sleep Show” for the last 3 years, heard every Friday at 1PM on KAAM 770 in the Dallas area. Vice President: Dr. Erin E. Elliott Dr. Erin Elliott is a practicing general dentist with a private practice in Post Falls, Idaho. She focuses on many aspects of dentistry including general and cosmetic dentistry as well as orthodontics and dental sleep apnea. Dr. Elliott is a past president of the American Sleep and Breathing Academy. Dr. Elliott has authored several articles on Dental Sleep Medicine, She is a regular contributor to Dental Economics including her latest article published in the October 2012 issue of Dental Economics entitled “Take the Time to Check for Sleep Apnea and “Pediatric Sleep Apnea” in the June 2013 issue of Dental Economics. She is considered a national expert in this growing field of dentistry and has lectured extensively educating dentists on how to incorporate dental sleep medicine into their practices. Credentialing Director: Dr. Paul Van Walleghem Dr. Paul Van Walleghem has been practicing Dentistry for 27 years and Dental Sleep Medicine for 22 years. Treasurer: Dr. Richard Klein DDS Dr. Richard Klein founded Michigan Head & Neck Institute, PC in 1985 to provide a place for people suffering from TMJ problems to seek specific treatment. After practicing general dentistry for 17 years (including the voluntary time he spent when he served as a Lieutenant in the US Naval Dental Corp), he saw a need to open a practice that exclusively treated patients with TMJ problems as he saw an increasing amount of patients with the relevant symptoms, and treatment for the condition was evolving.
