Inspired by his father’s battle with severe sleep apnea and guided by pioneers like Dr. Keith Thornton, he found his calling. With advanced training from respected institutions such as the Pankey Institute, UCLA, and the Pride Institute, Dr. Carstensen blended education, leadership, and clinical excellence into a path that has impacted thousands of lives. Reflecting on his career, Dr. Carstensen has witnessed the field evolve—but also acknowledges that there’s still work to be done. “We’re still too focused on managing adult symptoms with oral devices, and not enough on understanding and addressing the root causes, especially in children,” he says. One pivotal moment that shaped his clinical philosophy involved his own father, whose subjective improvement with a mandibular advancement device (MAD) didn’t align with objective testing results. “That taught me we can’t just go by how people feel. We need solid data to be responsible providers.” Dr. Carstensen also points out that many dentists miss critical signs of sleep apnea because they stop listening after hearing requests for a “snoring appliance.” True diagnosis and effective treatment, he says, require a deeper dive—into medical history, subtle symptoms, and comprehensive testing. When it comes to treating pediatric versus adult cases, Dr. Carstensen takes a collaborative and developmental approach. He works closely with pediatric dentists, orthodontists, speech pathologists, and myofunctional therapists to help children avoid the long-term effects of compromised breathing during growth. Adults, on the other hand, often require more invasive management due to years of damage already done. Looking ahead, Dr. Carstensen is particularly excited about how early dentists can now recognize underdevelopment of the craniofacial respiratory complex. “We’re starting to offer real, preventative solutions instead of reactive ones,” he says. He envisions a future where airway screening becomes as routine in dentistry as checking for cavities.

By Dr. Jerry Hu It seems there aren’t enough congratulatory words to describe how amazing and educationally enriching this year’s annual ASBA meeting in Houston was. It simply was a powerful and eye opening event. I have walked by and spoken to so many attendees, and they too had expressed this is the best dental sleep conference they had ever went to. A Paradigm-Shifting Opening Lecture First, the opening lecture with Dr. Timothy Adams had made the entire room have a paradigm shift. Thinking out of the box with quantum physics, biotensegrity, and fascia, he simply wowed the audience, and they left with so many “ah-hah” moments and pearls. Furthermore, it expanded their knowledge into TMJ-TMD and orofacial pain, as all of that, airway, apnea, distortions, and strains all had to do with posture. Teaching the dentists and allied health professionals about proper syntax and order of treatment was definitely eye opening for all who were present. It was indeed unforgettable. Day One: Introduction to Dental Sleep Medicine Next, Day one track for Introduction to Dental sleep medicine, taught by myself and Dr. Toshi Hart was unbelievably fantastic as well. It was fantastic because the attendees were alert and paying full attention. They asked very important questions and learned quickly and efficiently. The ones scheduled for the written exam to become board certified with ASBA Diplomate credentialing ALL passed their exam! The other amazing aspect is that they all know that education is a forever commitment. The attendees were all excited to sign up to ASBA’s study club, and they all want to pursue and enrich their own knowledge and treatment they can offer to their patients. They also expressed that they want to do pediatrics- kiddos as well, because they want to prevent a lot of airway and growth problems at as young of an age as possible. A Bright Future for Sleep Medicine So, you see, the positive energy, the meeting’s environment, the camaraderie and support from everyone there created a powerful aim to be reckoned with. Dr. Timothy Adams continues to amaze me with his depth of knowledge and his heart for this profession. I’m inspired by his leadership and vision, and how deeply he cares about helping patients. The way he presents and connects ideas always challenges me to think bigger and do better. I’ve been wanting to hear Patrick McKeown – Breathing Instructor and Best Selling Author live for years. His work with the Buteyko Breathing Method is so impactful, and it was a privilege to finally learn from him in person. Always great hearing from Dr. Steve Carstensen Carstensen! Nasal breathing is such an important piece of the puzzle, and he always brings it home in a practical, meaningful way. It was my first time hearing Dr. David Singh, and I left truly intrigued by his ideas and the way he blends structure, science, and innovation. Dr. Jerry Hu, DDS, DABDSM, DASBA, DACSDD, MICOI, AFAAID,LVIF brings a flood of insight, passion, and experience to the conversation, and his energy for this field is clear. And while Dr. Jon Caulfield didn’t speak, he presented the certificates and serves as president of the organization. It was great to connect with him in person. Dr. TOSHI HART, DDS also gave a beautiful, heartfelt presentation on pediatrics. She is such a valuable voice in this space. It was fun catching up with Rani Ben-David and John Nadeau. Always good to see familiar faces who’ve been in this field for years and continue to contribute. Missed getting a picture with Jon Nierman and Lane Rizzardini! However, it was great seeing them there! Meeting Mallory Love was another highlight. She’s one of the kindest souls, and I was so happy we finally got to connect in person. It was touching to hear about her personal journey and see her doing so well under Dr. Timothy Adams care. I also got to see Marietta Bibbs, from the Southern Sleep Society. She is an incredible leader and someone I’ve had the honor of SPEAKING for in the past. It’s always a joy to see doctors and dentists coming together for the greater good. And of course, I absolutely loved getting time with Dr. Sue Ellen Richardson DDS, D. ABDSM, FAASM. We’ve always shared a passion for learning and growing, and it’s just so energizing to be around her. She’s amazing. At the end of the event, I finally had the chance to receive my Diplomate certificate with the American Sleep and Breathing Academy IN PERSON. I actually earned it last year, but wasn’t able to attend the meeting to receive it. Being there to accept it this year brought a real sense of appreciation for the journey and the community we’re all building together. I left INSPIRED, GRATEFUL, and more curious than ever. In my entire career, I’ve never seen a more balanced team than the team of Dr. Jerry Hu, Dr. Tim Adams and myself. This weekend, we had a massive meeting with over 700 doctors (both MDs and DDSs). Through ASBA, we have it all! Dr. Tim Adams’ unique ability to take complicated concepts and simplify them allows us to understand the influence that quantum physics, epigenetics, biotensegrity, fascia, and mechanotransduction have on the treatment of our airway and TMJ patients. Understanding facial asymmetry and cranial strain patterns allows us to treat the root cause instead of just the symptoms that patients present to clinicians today. By introducing the open-loop Cricket dental appliance, Dr. Adams has given clinicians the desperately needed tools to allow the patient’s body to augment its natural cyclic intermittent motion and movement, create the spaces needed to allow cranial strain patterns to unwind, and restore the motion needed to allow the body to heal itself. Dr. Jerry Hu is documented by the US government as successfully achieving the lowest AHI with the Herbst appliance. He is a master of sleep treatment. I, Dr. Felix Liao, treat painless epigenetics with my Start Thriving appliance and my creation of AMDs (Airway Mouth Doctors). I will also add that we have other things in common—besides all being Diplomates of the American Sleep and Breathing Academy [1]. We also send all of our cases to Gergen’s Orthodontic Lab where the world’s greatest dental technician resides, David Gergen. In my opinion, nobody has done more for sleep medicine than David Gergen. David Gergen is an artist! Everything he does, he puts his heart and soul into it. His hands are magic; he cuts border movements into the Herbst Appliance and knows how to position the arms. Also, ask Dr. Tim Adams what David can do when he’s making Tim’s Crickets. The same applies to my Start Thriving appliances. David puts 100% into everything he does. I learned from Todd Huntsman that Gergen’s is also Vivo’s top lab as well. Jon Caulfield, the president of ASBA, only sends all of his cases to Gergen’s Orthodontic Lab because it’s the best. We also learned that American Sleep and Breathing Academy Diplomates scored the highest among all academies on lowering AHI levels. David Gergen is an ambassador from the state of Arizona to the President, helping with VA reform. Mark Brnovich, who is a member to ASBA Senior Advisory Board, was also nominated as a U.S. Ambassador to Serbia. Andre Collins from the NFL Players Association was also in attendance. I will say it again: I’ve never seen such a powerful organization. This organization has it all, and I’m extremely proud to be in the Hall of Fame and to serve as the Vice President of the ASBA! I joined ASBA last year upon recommendation from David Gergen, thinking to further my knowledge on the topic of sleep apnea. I read several of the articles on the ASBA website, and then signed up for the study club which is held periodically throughout the year. They have all been incredibly informative and worth the investment. The topics have spanned from sleep apnea concepts and etiologies to treatments with a range of appliances. Practice setup, medical billing and real life scenarios are some of the presentations I found especially helpful. Each seminar has been taught by the foremost pioneers and experts in the field, including Dr. Tim Adams, Dr. Jerry Hu, and Mr. Gergen himself. Each presentation has left me with a desire to do more OSA therapy as a dental practitioner, but more importantly, they have all given me the needed tools to DO, ACCOMPLISH and SUCCEED if I implement these principles.
